Goal: Double the number of older adults who complete an evidence-based chronic disease and/or diabetes self-management education training workshop.

Participating Organizations: CalOptima, Health Services Advisory Group, OC Health Care Agency, OC Office on Aging, Orange County Aging Services Collaborative, St. Jude Medical Center

24 Month Goals: July 2020 – June 2022

  1. COVID – 19 Vaccine Distribution
    • Support the efforts of the newly formed Older Adult Committee of the Vaccine Taskforce
    • Share Vaccine Distribution information throughout all channels within committee
    • Promote older adult vaccination education throughout the community
    • Support Older Adult’s vaccination plan
  2. Annual Wellness Tookit Support
    • Present at CalOptima’s Health Network Forums in the Spring
    • More providers need to know about Wellness Toolkit
    • Send electronically to providers and committee members in their networks
  3. Be Well Older Adult Workgroup Support
    • Create directory of community partner’s presentations to support older adult mental health in OC
    • Share the Be Well OC presentation with our organizations, clients and members
    • Provide feedback on the Be Well Older Adult presentation on mental health for older adults
  • Ongoing agenda item: Chronic Disease Self-Management Classes Reporting and Promotion

Quarter 1
January – March 31 2019

  • The subcommittee’s goal is to have 700 senior participants complete an evidence-based chronic disease self-management  workshop over the 18-month time frame. The subcommittee has had  179 completers to date
  • Seeking out potential partnerships to help with contributing numbers to  goal
    • Asian American Senior Citizens Service Center

  • Go4Life, an exercise and physical activity campaign from the National Institute on Aging at NIH is the materials that the subcommittee would like to use to promote
    • Working  with IrisOC team to add Go4Life campaign onto the IrisOC website

April – June 30 2019

  • The subcommittee’s goal is to have 700 senior participants complete an evidence-based chronic disease self-management  workshop over the 18-month time frame. The subcommittee has had  179  completers to date

  • ​The subcommittee’s goal is to have 700 senior participants complete an evidence-based chronic disease self-management workshop. The subcommittee has had 259 completers to date.
  • The subcommittee is also seeking out new potential partners to help with contributing numbers to the goal and have approached Asian American Senior Citizens Service Center.
  • An informational flyer is available in English, Spanish, Vietnamese, Korean, and Mandarin.
  • Classes are posted on the California Healthier Living Website.

Quarter 4
October – December 31, 2019

  •  259 older adults have attended and completed a chronic disease self-management workshop throughout Orange County.
  • The subcommittee has officially merged in December 2019 with OCASC’s Orange County Healthy Aging Initiative (OCHAI) Committee since their goals align with one another. The committee will be co-branded with both committee names.
